Abstract Objectives: Organ transplant is a complex process that involves various medical, legal, and social factors. The organ demand continues to outweigh the supply, leading to global challenges in the expansion of transplant programs. Ischemic time is crucial for the viability of organs, and efforts are being made to reduce transport time to improve transplant success rates. The viability and quality of the organ for transplant depends on minimized ischemic time.

Red blood cell distribution width (RDW)biomarker is a simple and inexpensive laboratory test that may be seen as a valuable perspective for initial patient evaluation. The consent rate and donor management in the cases of brain death is unacceptable. The coordinators need the training to increase their efficiency in terms of family approach and maintenance of brain death. Only by improving the level of family consent and increasing the coordinators' maintenance skills for brain death cases, the amount of organ donation in Iran can be doubled to the current amount.
